This case report describes a patient with locally recurrent cecal neuroendocrine carcinoma (NEC) after surgery who achieved a pathological complete response following treatment with a colorectal cancer–based chemotherapy regimen (FOLFOXIRI plus bevacizumab).

Although colorectal NEC is extremely rare and generally associated with a poor prognosis, this case highlights the potential importance of tumor biological characteristics and serum CEA levels in guiding therapeutic decision-making.
