This report describes an innovative approach to endoscopic sphincterotomy (EST) in a patient with situs inversus totalis, a rare condition characterized by mirror-image anatomy.
In such cases, precise control of the incision direction is technically challenging.
By using a rotatable sphincterotome, optimal blade orientation was achieved through extracorporeal manipulation alone, enabling safe and effective EST.
In addition, the procedure was successfully performed using the 360-degree turn technique without repositioning the patient or the endoscope, contributing to procedural stability.
This technique may represent a useful strategy for improving the safety and reliability of ERCP and EST in anatomically challenging cases.
